For returning Houghton College student-athletes, please complete the following steps to complete the necessary NCAA Compliance forms and Athletic Training medical documentation.
Please note, any questions about NCAA Compliance and Eligibility should be directed to Jason Mucher and anything related to Athletic Training and Medical should go to Deanna Hand.
1) Complete NCAA Compliance Forms
Once the NCAA makes their annual forms available, you will be sent an email providing access to and directions on how to complete the NCAA online forms. For fall athletes, this email will be arriving in mid- to late-June, and the forms must be complete prior to your arrival on campus in mid-August. You will not be able to participate in any team activities/practices until the forms are complete in the system. We will not accept any hard-copy forms this year. Winter and spring athletes will receive access to the forms prior to their first team practices in the fall.
2) Review the NCAA SUMMARY OF NCAA REGULATIONS
3) Complete an Annual Sports Physical and Medical Forms
All forms are available through
SportsWare. Please read through the
detailed instructions before proceeding to the SportsWare web portal.
- Returning student-athletes are required to update their Health History annually using the Pre-participation Physical Examination form and complete the required documentation (see SportsWare link above) prior to the first day of athletically related activity. No participation of any kind will be allowed without an updated Health History.
- The following additional Medical Eligibility Forms are also included at the link above and must be completed prior to participation:
- Assumption of Risk
- Mandatory Insurance Acknowledgment Form
- Release of Medical Information and Permission to Treat Minor Forms
- Concussion and Injury Statement
- Review Emergency Contact/Insurance Packet
- If applicable, update ADD/ADHD Medication and Banned Substance Documentation on the SportsWare portal.
- If you have been prescribed medication to treat ADD/ADHD or another medication that may be included on the NCAA list of banned substances, you must review and complete the following forms:

4) Attend a mandatory NCAA compliance meeting:
TEAM COMPLIANCE MEETINGS FOR FALL ATHLETES WILL BE SCHEDULED FOR WHEN YOU COME TO CAMPUS IN AUGUST:
These meetings are conducted by the athletic department's compliance officer. Pertinent NCAA bylaws, campus academic standards for eligibility, and compliance issues will be covered at this meeting. SA's will also have the opportunity to ask questions and receive clarification about NCAA bylaws and other issues regarding eligibility.
