New Athlete Compliance & Medical Forms

For student-athletes who have not competed on a Houghton University team in the past, please follow the following steps to complete the necessary NCAA Compliance forms and Athletic Training medical documentation.

Please note, any questions about NCAA Compliance and Eligibility should be directed to Jason Mucher and anything related to Athletic Training and Medical should go to Deanna Hand.

1) For recent graduate of a U.S./domestic high school or transfer who previously only attended U.S./domestic high school... Create an NCAA Profile and receive an NCAA ID. Once you are on the NCAA page, follow these steps:

STEP 1: After going to the NCAA Profile page, click the blue "Create a Profile Page" button at the bottom of the page.(Do NOT click the "Create an Account" button. That is for DI or DII).
NOTE, if you have already registered with the NCAA Clearinghouse and received an NCAA ID,
you can use that ID and skip to Step 3 below to fill out the Houghton NCAA Compliance Roster Form.
STEP 2: Enter your email address. This should be an address that you can access now and plan to use while at Houghton.
STEP 3: Click "Send"
STEP 4: Check your email for a verification code. The email will be sent from noreply@ncaa.org and titled NCAA Eligibility Center Verification Code. You may need to check your spam or junk folder.
STEP 5: Go back to the Eligibility Center website and enter your eight-digit verification code. Then select "continue". Write down your password in a place that is safe and easy to remember.
STEP 6: Enter your basic information. Name, race, gender, date of birth, high school graduation date, college enrollment date, and parents/guardians level of education. It may even ask for information on previous schools you attended (high school or other colleges).
STEP 7: Enter your contact information.
STEP 8: Enter the information for the sport(s) you plan to play at Houghton.
STEP 9: Confirm the Terms and Conditions, "Sign" and submit. NOTE: Once you get your NCAA ID, you can go to Step 3 and fill out the Houghton NCAA Compliance Roster Form.

2) International college-bound student-athletes (first-year enrollees and transfers) who initially enroll full time at an NCAA Division III school on or after Aug. 1, 2023, must have their amateur status certified by the NCAA Eligibility Center by creating an Amateurism Certification Account. (Academic documents may be requested to establish your official graduation timeline for amateurism certification purposes.)

I attend high school at a U.S. territory. Am I a domestic or international student?
  • Division III college-bound student-athletes who attended an international high school, competed outside of the United States or have permanent residence outside of the United States are considered "international" student-athletes for amateurism purposes and require an amateurism certification from the Eligibility Center. (This includes U.S. territories.) These students should create an Amateurism-Only Certification account (option 2 on eligibilitycenter.org).
  • Best practice for all Division III student-athletes: Please consult the NCAA school recruiting you before creating either Certification account to ensure you are creating the one that meets the specific requirements of the recruiting school.
  • College-bound student-athletes who have attended high school at a U.S. high school are considered "domestic" student-athletes and are not required to register with the Eligibility Center. Best practice is to create a free Profile Page account (option 1 on eligibilitycenter.org).

3) Fill out the NCAA Compliance Roster Form.

Once this form is complete, a team roster will be created and each team member will be sent a follow-up email providing access to and directions on how to complete the NCAA compliance forms. Please note that the follow-up email will not come right away, so be patient. The email will be sent once the NCAA forms are released in mid-July. The forms must be complete prior to your arrival on campus in mid-August. You will not be able to participate in any team activities/practices until the forms are complete in the system. We will not accept any hard-copy forms this year. Winter and spring athletes will receive access to the forms prior to their first team practices in the fall.

Once you are in the system and are added to a team roster, future years will only require us to email you the link to the NCAA Compliance Forms for the upcoming year.

5) Complete an Annual Sports Physical and Medical Forms

All forms are available through SportsWare. Please read through the detailed instructions before proceeding to the SportsWare web portal.
  • All student-athletes are required to have a pre-participation physical exam and complete the required documentation (see link above) prior to the first day of athletically related activity. No participation of any kind will be allowed without a physical. Before going to the doctor for a physical, you should complete the history portion of the form. You should review the history with the examining physician at the time of the physical exam. The form requires a physician's signature and, once completed should clearly indicate your medical clearance status for all the sports you expect to participate in during the course of the academic year (indicate these sports at the top of the pre-participation form). Please have your examining medical provider clearly document their name, address and phone number. For fall preseason athletes, completed forms need to be returned to the Student Health Center prior to August 1.
  • New student physicals required for admission to the college will count toward the student-athlete sport physical, but student-athlete will need to complete the Medical History form prior to participation. These exams must also have occurred with the six months prior to athletic participation.
  • Go the Houghton Health Center website to complete information concering CMR/Immunizations forms.
  • The following additional Medical Eligibility Forms are also included at the link above and must be completed prior to participation:
    • Sickle Cell Trait: All student-athletes are required to show proof of Sickle Cell Trait bloodwork results (Infant testing will be accepted) and complete the Sickle Cell Trait form, also found on SportsWare. Student-athletes who do not provide results will need to complete an online education session (link will be sent to the student-athlete via email) and complete testing or provide results within 10 days from the first date of practice. After 10 days, the student-athlete will be considered medically ineligible to participate if results have not been received. Learn more about Sickle Cell Trait.

    • Mandatory Insurance Acknowledgment Form
    • Release of Medical Information and Permission to Treat Minor Forms
    • Concussion and Injury Statement
  • ADD/ADHD Medication and Banned Substance Documentation are available on the SportsWare portal
    • If you have been prescribed medication to treat ADD/ADHD or another medication that may be included on the NCAA list of banned substances, you must review and complete the following forms:

6) Attend a mandatory NCAA compliance meeting:

TEAM COMPLIANCE MEETINGS FOR FALL ATHLETES WILL BE SCHEDULED FOR WHEN YOU COME TO CAMPUS IN AUGUST:

These meetings are conducted by the athletic department's compliance officer. Pertinent NCAA bylaws, campus academic standards for eligibility, and compliance issues will be covered at this meeting. SA's will also have the opportunity to ask questions and receive clarification about NCAA bylaws and other issues regarding eligibility.
